Job summary

Molina Healthcare in Naperville, IL is seeking a Care Coordination Specialist to support LTSS activities. You will collaborate with a multidisciplinary team to coordinate member care across the continuum, monitor progress toward outcomes and contribute to quality, cost‑effective services.

Requirements include at least 2 years in health care with care management experience, active licensure (LCSW/LMSW/APSW/LPC/LPCC/LMFT/PhD or PsyD) and a valid driver’s license with willingness to travel 25–40%.

Qualifications

• At least 2 years in health care with care management and LTSS exposure.
• Active licensure as listed (LCSW/LMSW/APSW/LPC/LPCC/LMFT/PhD/PsyD) or equivalent.

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including home visits as required.
• Facilitates waiver enrollment/disenrollment.
• Develops and implements care plans with members and care network.
• Monitors care plan effectiveness and documents progress.
• Promotes integration of LTSS and behavioral health services to improve continuity of care.
• Assesses medical necessity and authorizes waiver services.
• Identifies barriers to care and helps address psycho/social, financial, and medical obstacles.
• Travel 25-40% may be required based on state/contractual requirements.
