Job summary

Molina Healthcare is seeking a Care Manager for LTSS coordination in Illinois. You will collaborate with multidisciplinary teams to implement member care plans, conduct comprehensive assessments, and monitor progress toward outcomes.

Travel 25-40% may be required based on state contracts. Strong communication, problem-solving and knowledge of community resources are essential to navigate waiver services and care coordination across settings.

Qualifications

• 2+ years of health care experience, LTSS experience preferred
• Clinical licensure/certification as required by state contract/regulation
• Bachelor's degree may be required in some states
• Valid driver's license and reliable transportation
• Demonstrated knowledge of community resources
• Ability to work independently with minimal supervision

Responsibilities

• Complete comprehensive member assessments within regulated timelines and during home visits
• Facilitate waiver enrollment/disenrollment processes
• Develop and implement care plans with members and care teams
• Monitor care plan effectiveness and adjust as needed
• Promote integration of LTSS with behavioral health and community resources
• Identify barriers to care and develop prevention plans
