Job summary

Molina Healthcare is seeking a Care Coordinator for LTSS-focused care management in Illinois, including Jerseyville. The role collaborates with a multidisciplinary team to assess, plan, and coordinate member care across settings, with travel to member homes as needed.

You will develop and monitor comprehensive care plans, coordinate waiver services, and promote integration of behavioral health and LTSS resources to support quality outcomes. Local travel up to 40% is expected.

Qualifications

• At least 2 years of health care experience, including at least 1 year with LTSS and 1 year in care management.
• License or certification required only if state contract mandates it; active and unrestricted if licensed.

Responsibilities

• Completes comprehensive member assessments within timelines, including home visits.
• Develops and implements care plans with members, caregivers, physicians and others.
• Monitors care plan effectiveness, documents interventions and adjusts as needed.
• Promotes integration of behavioral health, LTSS, and community resources.
• Assesses medical necessity and authorizes waiver services; coordinates ICT meetings.
• Identifies barriers to care and provides coordination to address psycho/social, financial, and medical obstacles.
• 25-40% local travel may be required.
