Job summary

Molina Healthcare is seeking a Care Management/Coordinator in Illinois to support LTSS and care coordination for high-need members. You will work with a multidisciplinary team to ensure timely assessments, plan implementation, and ongoing monitoring to improve outcomes and cost efficiency.

The role involves waiver services coordination, travel up to 40% locally, and collaboration with physicians, caregivers, and other professionals.

Qualifications

• 2+ years healthcare experience, including LTSS/care management or equivalent.
• Licensure/certification required only if state contract dictates.
• Bachelor’s degree may be required in some states.

Responsibilities

• Complete comprehensive member assessments within required timelines, including home visits when needed.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, caregivers, physicians and care team.
• Monitor care plans and document interventions and outcomes.
• Promote integration of services across LTSS, behavioral health and community resources.
• Assess medical necessity and authorize waiver services.
• Evaluate benefits and advise on funding sources.
• Facilitate ICT meetings for service approvals/denials.
• Educate and motivate members using motivational interviewing.
• Identify barriers to care and coordinate solutions.
• Identify incidents and develop prevention plans.
• Collaborate with licensed care managers as needed.
• Travel 25–40% may be required.
