Job summary

Molina Healthcare is seeking a care management professional in Illinois to coordinate LTSS-enabled member care across settings. You will conduct assessments, develop and monitor care plans, and collaborate with physicians, caregivers and the care team to meet member goals.

Travel up to 25-40% may be required to support waiver enrollment, home visits and service coordination across the continuum. Strong communication and organizational skills are essential.

Qualifications

• Minimum 2 years health care experience with LTSS exposure.
• Valid driver's license and reliable transportation for travel.
• Ability to work independently in diverse settings.

Responsibilities

• Completes comprehensive member assessments within regulated timelines.
• Develops and implements care plans in collaboration with teams and members.
• Promotes integration of LTSS services and community resources.
• Monitors care plan effectiveness and documents interventions.
• Facilitates ICT meetings for approval or denial of services.
