Job summary

Molina Healthcare is seeking a care coordination professional to support LTSS members across multiple Ohio counties. You will collaborate with a multidisciplinary team, conduct comprehensive assessments (including home visits), develop care plans, and monitor outcomes to ensure quality and cost-effective member care.

The role emphasizes coordination of LTSS and behavioral health integration, with travel required to various counties and a focus on member-centric care delivery within regulatory

Qualifications

2+ years health care experience (LTSS/care management)
Licensure/certification required only if state mandates
Valid driver’s license with reliable transportation
Knowledge of community resources
Strong communication and time management

Responsibilities

Complete comprehensive member assessments within timelines, including home visits
Facilitate waiver enrollment/disenrollment processes
Develop and implement care plans with members and care network
Monitor care plan effectiveness and document outcomes
Promote integration of LTSS and behavioral health services
Assess medical necessity and authorize waiver services
Collaborate with ICT meetings for service decisions
Travel up to 25-40% as required
