Qualifications

• At least 2 years of health care experience, incl. LTSS and care management.
• Licensing only if state contract requires; active and unrestricted.
• Bachelor's degree may be required in some states.
• Valid driver's license, reliable transportation, and travel readiness.
• Knowledge of community resources and ability to work with diverse populations.
• Strong communication and organizational skills.
• Experience with substance use disorders is beneficial.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitate comprehensive waiver enrollment and disenrollment processes.
• Develop and implement care plans, including a waiver service plan with the member and care network.
• Monitor care plans, document interventions, and adjust as needed.
• Promote integration of LTSS, behavioral health, and community resources.
• Assess medical necessity and authorize services for waivers.
• Evaluate benefits and advise on funding sources and options.
• Facilitate ICT meetings for service approval/denial and collaboration.
• Use motivational interviewing to educate and motivate member change.
• Identify barriers to care and coordinate psycho/social, financial, and medical support.
• Identify critical incidents and develop prevention plans.
• Collaborate with licensed care managers as needed.
• Travel 25-40% depending on state/contractual requirements.
