Job summary

Molina Healthcare is seeking a care coordination professional to support LTSS members across multiple counties in Ohio. The role focuses on comprehensive assessments, care planning, and coordination with licensed professionals to deliver high-quality, cost-effective member care.

Responsibilities include coordinating waiver services, monitoring plans, and ensuring timely benefits authorization while addressing psycho-social and financial barriers.

Qualifications

• Minimum 2 years health care experience with LTSS, disability, or chronic conditions.
• 1 year care management or medical/behavioral health experience.
• Active license/certification only if required by state contract or regulation.
• Bachelor’s degree may be required in some states.
• Valid driver’s license and reliable transportation for travel.
• Strong knowledge of community resources and ability to work across settings.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including home visits as needed.
• Facilitate waiver enrollment/disenrollment processes.
• Develop and implement care plans with members and care network.
• Monitor care plans and adjust interventions to meet goals.
• Promote service integration across behavioral health, LTSS, and community resources.
• Assess medical necessity and authorize appropriate waiver services.
• Facilitate ICT meetings for service decisions and collaboration.
• Identify barriers to care and address psycho/social, financial, and medical obstacles.
