Qualifications

• 2+ years healthcare experience, LTSS and care management experience preferred.
• Licensure/certification required only if state contract mandates it; active in state of practice if licensed.
• Bachelor's degree in a healthcare field may be required in some states.
• Valid driver's license and reliable transportation for travel requirements.
• Proven ability to work with diverse populations and in dynamic settings.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, caregivers, physicians and support network.
• Monitor care plan effectiveness and adjust interventions as needed.
• Promote integrated services across behavioral health, LTSS, and community resources.
• Assess medical necessity and authorize waiver services.
• Collaborate in interdisciplinary care team meetings for approvals.
• Educate and motivate members using motivational interviewing and clinical guides.
• Identify barriers to care and coordinate resources to address psycho/social and financial obstacles.
• Identify critical incidents and develop prevention plans to protect member health.
