Job summary

Molina Healthcare is seeking a Care Coordinator to support LTSS-focused care management across multiple Ohio counties. You will coordinate comprehensive assessments, develop and monitor care plans, and work with a multidisciplinary team to align services with member goals.

Travel to applicable counties is required 25-40%. Strong communication, organization, and problem-solving skills are essential to ensure high-quality, cost-effective member care across settings.

Qualifications

• At least 2 years health care experience incl LTSS and care management.
• Valid driver's license and ability to travel 25-40%.
• Clinical licensure/certification required if state contract mandates.
• Experience with disability/chronic conditions is a plus.
• Strong verbal and written communication skills.

Responsibilities

• Complete comprehensive member assessments within regulated timelines.
• Facilitates comprehensive waiver enrollment and disenrollment processes.
• Develops and implements care plans in collaboration with members and care network.
• Monitors care plan effectiveness and adjusts as needed.
• Promotes integration of LTSS, behavioral health, and community resources.
• Identifies barriers to care and coordinates support for psycho/social, financial, medical needs.
• Travel required 25-40% depending on state/contract.
