Job summary

Molina Healthcare is seeking a Care Management Coordinator to support LTSS activities across multiple counties in Ohio. You will work with a multidisciplinary team to coordinate member care, develop and monitor care plans, and promote integration of services to improve outcomes.

The role requires 2+ years in health care, LTSS or care management, a valid driver’s license, and strong communication skills. Travel up to 40% is expected and you will collaborate with licensed care managers as needed.

Qualifications

• 2+ years in health care, LTSS, or care management.
• Valid driver's license, reliable transportation, and adequate auto insurance.
• In some states, a health care degree may be required by contract or regulation.
• Strong communication and ability to work with diverse populations.

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including in-person home visits when required.
• Facilitates comprehensive waiver enrollment and disenrollment processes.
• Develops and implements care plans with members, caregivers, and providers to address needs and goals.
• Performs ongoing monitoring of care plans and documents progress and interventions.
• Promotes integration of LTSS and behavioral health services and coordinates with ICT.
