Job summary

Molina Healthcare is seeking a care management professional in Ohio to support LTSS coordination and member outcomes. You will work with a multidisciplinary team to develop and implement care plans, coordinate waiver services, and promote integrated care across settings.

Responsibilities include assessing needs, monitoring outcomes, and navigating barriers to care, with travel required to various service areas. A healthcare background with LTSS or disability experience is preferred.

Qualifications

• 2+ years in healthcare, LTSS/CARE mgmt experience preferred
• Licensed/certified if required by state contract or regulation
• Valid driver's license and reliable transport for travel requirements

Responsibilities

• Complete comprehensive member assessments within timelines, including in-person visits
• Coordinate waiver enrollment and disenrollment processes
• Develop and implement care plans with members, families and clinicians
• Monitor care plan effectiveness and adjust as needed
• Promote integration of LTSS and behavioral health services
• Assess medical necessity and authorize waiver services
• Collaborate with care managers to ensure quality care
