Job summary

Molina Healthcare is expanding its care management team in Ohio to support LTSS members across multiple counties. The role collaborates with a multidisciplinary team to coordinate integrated care, ensuring progress toward outcomes while balancing quality and cost-effectiveness.

Responsibilities include completing assessments, coordinating waiver enrollment, developing and monitoring care plans, and facilitating interdisciplinary team meetings.

Qualifications

• At least 2 years of health care experience with LTSS settings.
• Licensure or certification only if required by state contract or licensing mandates.
• Some states may require a healthcare-related bachelor's degree.
• Valid driver’s license, transportation, and adequate auto insurance for travel.
• Knowledge of community resources and ability to work with diverse populations.
• Strong written and verbal communication and problem-solving skills.
• Experience with substance use disorders in the last year is a plus.
• Proficiency with Microsoft Office and online portals/databases.

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitates waiver enrollment and disenrollment processes.
• Develops and implements care plans with members, caregivers and clinicians.
• Monitors progress, documents interventions, and suggests plan changes.
• Promotes service integration across behavioral health, LTSS, and community resources.
• Assesses medical necessity and authorizes waiver services.
• Evaluates benefits and advises on funding sources.
• Facilitates ICT meetings for approval/denial of services.
• Uses motivational interviewing to educate and motivate member change.
• Identifies barriers to care and coordinates psycho/social, financial, and medical support.
• Identifies critical incidents and develops prevention plans.
• Travels 25-40% locally as required.
