Qualifications

• At least 2 years healthcare experience, incl 1 year LTSS/care management or equivalent.
• Valid driver’s license and reliable transportation.
• Experience with community resources.
• Ability to work independently with minimal supervision and in high-pressure settings.
• In some states, must have experience with individuals with substance use disorders.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in‑person home visits.
• Facilitate comprehensive waiver enrollment and disenrollment processes.
• Develop and implements care plans with members, caregivers, physicians and care team.
• Monitor care plans to evaluate effectiveness and adjust as needed.
• Promote integration of LTSS and behavioral health services and connect to community resources.
• Assess medical necessity and authorize waiver services.
• Facilitate ICT meetings for approvals/denials.
• Travel 25–40% as required.
