Job summary

Centene is seeking a Care Manager to support Illinois Health Plan members through holistic long-term support and care coordination. The role involves developing personalized service plans and educating members and caregivers about available services and benefits.

The position blends remote work with approximately 50% local travel for in-home visits, serving members with physical disability and aging waiver.

Qualifications

• Requires Bachelor's degree and 1 year related experience.
• RN license in Illinois is preferred.
• LPN with 1 year experience in assessments acceptable.

Responsibilities

• Evaluates the needs of the member, the resources available, and recommends and/or facilitates the plan for the best outcome.
• Assists with developing ongoing long-term care plans/service plans and identifies providers, specialists, and community resources.
• Coordinates between member/family/caregivers and care provider team to ensure services are accessible.
• Provides resource support for employment, housing, participant direction, independent living, justice, foster care.
• Monitors care plans/service plans, member status and outcomes, and provides recommendations.
• Interacts with long-term care providers and partners to ensure member needs are met.
• Collects, documents, and maintains long-term care member information for compliance.
• May perform on-site visits to assess member's needs and collaborate with providers.
• Provides education to long-term care members and families on procedures, options, and benefits.
• Provides feedback to leadership on opportunities to improve care and delivery.
• Performs other duties as assigned.
• Complies with all policies and standards.

Bachelor's degree
