Job summary

Qualifications

• Bachelor's degree and 1 year related experience or equivalent.
• Case management experience with waiver populations.
• Strong understanding of community-based services and care coordination.
• Willingness to travel up to 50% within Illinois.
• Knowledge of long-term care services and regulatory requirements.

Responsibilities

• Evaluate member needs and resources to design optimal care plans.
• Develop and monitor long-term care plans and identify providers.
• Coordinate between members, families, and care teams to ensure service access.
• Provide resources and education to members and families on services and benefits.
• Monitor plan outcomes and adjust as needed.
• Liaise with long-term care providers to meet member needs.
• Document member information to comply with state, federal, and payer rules.
• Perform on-site visits as needed.
• Provide leadership feedback to improve care quality.
• Adhere to policies and standards.
