Job summary

Centene is seeking an LTSS Service Care Coordinator in Schaumburg, IL to develop, assess, and coordinate holistic care management activities for quality, cost-effective outcomes.

The role requires evaluating member needs, developing long-term care plans, and coordinating with care providers. Expect on-site visits, education for members and families, and compliance documentation. 50% local travel and a full-time day shift are included.

Qualifications

• Bachelor's degree or nursing licensure with care coordination experience.
• Minimum 1 year of related experience.

Responsibilities

• Evaluate the member's needs and recommend/facilitate the best outcome plan.
• Assist with developing ongoing long-term care/service plans.
• Coordinate between the member and care provider team to ensure service accessibility.
• Provide resource support for various member needs based on service assessment.
• Monitor care plans and member status, providing recommendations as needed.
• Interact with long-term care healthcare providers to meet member needs.
• Collect, document, and maintain member information for compliance.
• Perform on-site visits to assess member needs and collaborate with providers.
• Facilitate education for members and families on healthcare options and benefits.
• Provide feedback to leadership on opportunities to improve care quality.

Education

Bachelor's degree

Bachelor's or Master’s Degree in a human services related field

RN license (Illinois)

LPN license with one year of elderly assessments experience
