Job summary

Candidates must have an Illinois RN license or a Bachelor/Master’s in a human services field, or be an LPN with relevant assessment experience.

Qualifications

• Experience with case management or home visits preferred.
• Bachelor/Master’s in human services fields defined in the candidate criteria.

Responsibilities

• Evaluate member needs and resources to shape care plans.
• Coordinate between member, family/caregivers and care team to ensure services are accessible.
• Provide education and resources on procedures, services and benefits.
• Monitor plans and outcomes; adjust as needed based on member needs.
• Ensure regulatory compliance and may perform on-site assessments as appropriate.
