Job summary

Texas Health and Human Services Commission (HHSC) seeks a LTC Services Screener II to interview elderly and disabled clients applying for Long Term Care Services.

The role processes CCSE referrals, provides information about services, and makes referrals to other programs and community organizations. Requires high school diploma and willingness to travel; bilingual English/Spanish preferred.

Qualifications

• Ability to communicate effectively in writing and speaking.
• Ability to set priorities, timelines, and meet deadlines.
• Ability to establish and maintain effective relationships with clients, staff, and partner agencies.
• Proficiency with computers and general office equipment.
• Proficiency using automated software (Microsoft Office, SharePoint, etc.).
• Knowledge of interviewing techniques to obtain information and resolve conflicts.
• Knowledge of resources serving older adults or individuals with disabilities.

Responsibilities

• Answers the Statewide toll-free Intake Line for CCSE.
• Maintains regular and predictable attendance.
• Follows HHS policies and procedures related to job performance.
• Accepts and processes telephone and fax requests for CCSE program services.
• Provides general and specific information about CCSE program services.
• Refer intakes to CCSE staff.
• Refer individuals to other programs, agencies, and community organizations.
• Completes special assignments and/or reports.
• Operates a computer to document, update or inquire into case records.
• Handles high-volume incoming calls in a Call Center environment.
• Communicates at a basic level with others to provide, exchange or verify information.
• Addresses inquiries, issues, or problems or complaints.
• Documents calls for management information on program operations.
