Job summary

Texas Health and Human Services Commission (HHSC) Community Services division seeks a Community Care Worker I to determine eligibility for CCSE services, conduct home visits, and connect Texans with in-home care and social supports. Qualifications

• High school diploma or equivalent required.
• Willingness to travel at least 65% of the time.
• Experience using email and other automated applications (e.g., Microsoft Office).

Responsibilities

• Attends regular and predictable schedule and performs other duties as assigned.
• Interviews applicants and conducts home/site visits to assess need for services.
• Verifies income and resources to determine financial eligibility.
• Maintains case records and enters information in automated systems.
• Monitors services to ensure needs are met.
• Develops and coordinates service plans and authorizes services accordingly.
• Prepares simple narratives or statistical reports.
