Job summary

The Texas Health and Human Services Commission (HHSC) seeks a Community Care Worker I in Mission, TX to determine eligibility for Community Care Services Eligibility (CCSE) program services. You will conduct home visits, interview seniors and individuals with disabilities, document information, and develop service plans, ensuring authorizations are properly recorded in automated systems.

Responsibilities

• Attends work on a regular schedule and performs other duties as assigned.
• Interviews applicants/recipients and conducts visits to assess service needs and provide information/referrals.
• Determines eligibility and financial eligibility for Title XX programs.
• Maintains case records and documents interactions in automated systems.
• Monitors services to ensure they meet the individual's needs.
• Develops or reviews service plans with individuals and families and authorizes services.
• Prepares basic narratives or statistical reports.
