Job summary

Responsibilities

• Attends work on a regular and predictable schedule per agency policy.
• Interviews applicants and conducts home visits to assess needs and provides information and referrals.
• Assesses eligibility and functional needs; verifies income/resources for Title XX programs.
• Maintains case records and enters data into automated systems.
• Monitors services by contacting clients to ensure needs are met.
• Coordinates, reviews, and authorizes service plans to meet client needs.
• Prepares basic narratives or statistical reports as required.
• Demonstrates knowledge of program requirements and policy regulations.
