Job summary

Haven Home Care in Eastern Ohio is expanding services and seeking an LPN for a supervisory, per-visit role. You will conduct supervisory visits, complete open visits for new clients, and handle pre-employment testing while traveling across Ashtabula, Columbiana, Mahoning, and Trumbull counties.

This expansion role offers flexible hours with no fixed weekly schedule, and mileage is reimbursed. Prior home health experience and Ohio LPN licensure are required.

Qualifications

• Must be at least 18 years old.
• Graduation from an accredited School of Practical Nursing.
• Current Ohio LPN license in good standing.
• Valid driver’s license and auto insurance for work use.
• Current CPR certification.
• Criminal background check completed.
• Residence in Trumbull or Mahoning County preferred.

Responsibilities

• Complete open visits and document accurately according to agency standards.
• Conduct supervisory visits every 60 days to check on Home Health Aides.
• Perform pre-employment testing for Home Health Aide candidates.
• Travel to client homes across Ashtabula, Columbiana, Mahoning, and Trumbull counties.
