Job summary

Haven Home Care is expanding its proven home care model into Eastern Ohio and seeks an LPN for a supervisory, per-visit role. You will complete open visits, supervise Home Health Aides every 60 days, and run pre-employment testing for aides.

The position offers flexible hours with growth as client sites develop across Ashtabula, Columbiana, Mahoning, and Trumbull counties. Mileage is reimbursed, and a current Ohio LPN license is required.

• Ability to work independently and schedule visits efficiently.

Responsibilities

• Prepare accurate and thorough documentation according to agency standards and regulatory requirements.
• Conduct regular supervisory visits every 60 days to check on Home Health Aides and complete open visits for new clients.
• Conduct skills tests and written tests for Home Health Aide candidates.
