Job summary

Haven Home Care is expanding in Eastern Ohio and seeks a Supervisory LPN to establish our presence across Ashtabula, Columbiana, Mahoning, and Trumbull counties. You will perform supervisory visits, complete open visits for new clients, and conduct pre-employment testing for aides, with emphasis on documentation and regulatory compliance.

This role is per-visit and expansion-focused rather than a traditional hourly position, offering flexible scheduling as the client base grows and new

Qualifications

• Graduation from an accredited School of Practical Nursing is required.
• Current Ohio LPN license in good standing.
• Valid driver’s license with reliable transportation; auto insurance for work.
• Current CPR certification.
• Criminal background check, skills test, and written nursing test completed.

Responsibilities

• Complete open visits and collect documentation according to agency standards.
• Conduct supervisory visits every 60 days to check Home Health Aides and progress client care.
• Perform pre-employment testing for Home Health Aide candidates.
