Amedisys, Inc. is seeking an organized Intake/Coordination professional to schedule home visits for field staff and coordinate patient care under the Clinical Manager.

This full-time role focuses on efficient use of clinical resources and timely communication with patients and clinicians. Responsibilities include managing clinician schedules, optimizing geography-based deployments, tracking orders and services, and supporting transfers between care centers as needed.

Formación

• Current, unencumbered license to practice as a LPN/LVN or RN in the state of employment.
• High School diploma or GED equivalent.
• One (1+) year of administrative experience in a health care environment.

Responsabilidades

• Schedule home visits for field staff to ensure timely patient care.
• Prepare weekly clinician schedules and monitor capacity to meet productivity standards.
• Assign clinicians efficiently by geography and skill to maximize care delivery.
• Ensure missed visits are rescheduled or referred to the Clinical Manager.
• Communicate with patients, field staff and Clinical Manager regarding scheduling of services.
