Job summary

AMEDISYS HOME HEALTH in Pulaski, VA is seeking a Clinical Patient Services Coordinator (LPN required). The role centers on scheduling home visits, coordinating patient care, and optimizing clinical resources to maximize center capacity and utilization guidelines.

Under the Clinical Manager, you will schedule field staff, communicate with patients and clinicians, monitor visits, and support transfer of patients between care centers.

Qualifications

• Current, unencumbered license to practice as a Licensed Practical Nurse/Licensed Vocational Nurse or Registered Nurse in the applicable state.
• High School diploma or GED equivalent.
• One year or more of administrative experience in a health care environment.

Responsibilities

• Schedules home visits for field staff and coordinates patient care with the Clinical Manager.
• Prepares weekly clinician schedules and reviews schedules to meet capacity and productivity standards.
• Assigns clinicians efficiently by geography and discipline, and ensures visits are completed as ordered.
• Manages calls from patients and field staff regarding scheduling issues and refers issues to the Clinical Manager.
• Monitors hospitalized patients and ensures follow-up as needed.
• Supports timely communication with patients, clinicians, referral sources and office staff.
• Completes required orders and services (lab draws, add-on visits, etc.).
• May provide direct patient care in emergencies if licensed and qualified.
