Job summary

Amedisys is seeking an Intake/Scheduling Coordinator to organize home visit schedules for field staff and coordinate patient care under the Clinical Manager. You will ensure efficient use of resources and clear communication with patients, field staff, and clinicians to maximize care center capacity.

The role requires an active LPN/RN or LPN/LVN license in the state of assignment, a high school diploma, and at least one year of healthcare administration experience.

Qualifications

• Active LPN/LVN or RN license for the state of assignment.
• High school diploma or GED required.
• 1+ year of administrative healthcare experience.

Responsibilities

• Schedules patient assessments and Oasis visits in a timely manner.
• Prepares clinician schedules and monitors capacity and productivity.
• Allocates clinicians to geographies to maximize efficiency and skills match.
• Manages missed visits and coordinates with Clinical Manager for resolution.
• Communicates scheduling issues to patients and field staff; monitors follow-ups.
• Supports transfer of patients between care centers or hospice services.
• May provide direct patient care in emergencies if licensed and needed.
• Maintains supply areas and assists with intake when needed.
• Performs other duties as assigned.
