Job summary

Amedisys is hiring an LPN Patient Service Coordinator Scheduler to manage home-visit scheduling for field staff and coordinate patient care under the Clinical Manager. The role requires a current LPN/LVN or RN license and at least one year of administrative experience in a healthcare setting.

Work hours are Monday through Friday, 8:00 am to 5:00 pm, with compensation of $30.00–$32.00 per hour. Prior scheduling experience in healthcare is preferred but not required.

Qualifications

• Current, unencumbered LPN/LVN license or RN license for the state of employment.
• High School diploma or GED.
• One year of administrative experience in a health care environment.
• Preferred: One year scheduling experience in healthcare.

Responsibilities

• Ensures patient assessment visits including Oasis visits are scheduled and performed timely.
• Prepares weekly clinician schedules and reviews daily to optimize capacity.
• Allocates clinicians efficiently by geography and skill for required care.
• Schedules missed visits or refers to Clinical Manager for resolution.
• Manages calls from patients and field staff about scheduling issues.
• Monitors hospitalized patients and ensures follow-up by the team.
• Completes requested schedules including orders and services as required.
• Supports Clinical Manager with timely communication to patients and staff.
• Runs reports and responds to workflow with appropriate actions.
• Assists with transfer of patients between care centers or Hospice services.
• May provide direct patient care in emergencies if licensed and needed.
