Amedisys is seeking an Intake Coordinator to schedule home visits for field clinicians and coordinate patient care under the Clinical Manager. You will manage scheduling, communications with patients and staff, and ensure efficient use of clinical resources within care center guidelines.

Responsibilities include preparing weekly schedules, monitoring capacity, resolving scheduling issues, and supporting transfers between care centers.

Qualifikationen

• Current, unencumbered license to practice as an LPN/LVN or RN in the state of assignment.
• High School diploma or GED equivalent.
• One or more years of administrative experience in a health care environment.
• One or more years of scheduling experience in a healthcare environment.

Aufgaben

• Schedules home visits for field staff and coordinates with patients and Clinical Manager.
• Prepares weekly clinician schedules and reviews capacity to meet productivity standards.
• Ensures clinicians are assigned efficiently by geography and discipline as needed.
• Resolves missed visits by week end or escalates to Clinical Manager.
• Manages scheduling calls from patients and field staff and supports transfer processes.
• Monitors patient status and ensures follow-ups are completed as ordered.
