Qualifications

• Administrative experience in a health care environment.
• Scheduling experience in a healthcare environment preferred.
• One year+ administrative experience in a health care setting.
• Scheduling experience in healthcare preferred.

Responsibilities

• Schedules home visits for field staff and coordinates patient care.
• Prepares weekly clinician schedules and reviews capacity to meet productivity standards.
• Assigns clinicians efficiently by geography and care requirements.
• Ensures missed visits are rescheduled or referred toClinical Manager.
• Collaborates with field clinicians to ensure all visits are completed as ordered.
• Manages calls from patients and field staff regarding scheduling issues.
• Monitors patient status and ensures follow-up as needed.
• Compiles schedules with required orders and services.
• Supports timely communication with patients, clinicians, referral sources and BD team.
• Runs reports and performs scheduling-related actions.
• Assists with patient transfers between care centers or hospice services.
• May provide direct patient care in emergencies if licensed and needed.
• Maintains supply closet and liaises with field for patient supply needs.
• Handles referral and intake processes when Intake Coordinator is absent.
• May obtain verbal orders for patients as applicable.
