Job summary

Community Care, Inc. is seeking a compassionate LPN to provide 1:1 private duty nursing care for a 19‑year‑old male child in Johnstown, PA. This full-time role offers evening hours (4:00 PM - 11:00 PM) on weekdays with weekends available, in a supportive home setting.

You will assist with ADLs, monitor seizures, manage g‑tube care and medications, and collaborate closely with the family and care team to deliver high-quality pediatric care.

Qualifications

• Current LPN (or RN) license in PA
• Graduate of an accredited nursing program
• At least 6 months of nursing experience (home health preferred)
• CPR certification

Responsibilities

• Perform g-tube care/feedings/aspiration precautions
• Monitor seizures
• Administer medications safely and accurately
• Communicate clearly and compassionately with the care team and family
