Job summary

Community Care, Inc. Home Health is seeking a compassionate LPN for 1:1 private duty pediatric care in Johnstown, PA. This full-time role provides care for a 19-year-old male child, Monday–Friday 4:00 PM–11:00 PM, focusing on ADLs, medication administration, and safety in a home setting.

You will join a supportive team, performing g-tube care, monitoring seizures, and delivering family-centered care with CPR-certified readiness.

Qualifications

• Current LPN (or RN) license in Pennsylvania.
• Graduate of an accredited nursing program.
• At least 6 months of nursing experience (home health preferred).
• CPR certification.

Responsibilities

• Assist with Activities of Daily Living (ADLs).
• Perform g-tube care/feedings/aspiration precautions.
• Monitor seizures.
• Administer medications safely and accurately.
• Communicate clearly and compassionately with the care team and family.
