Job summary

Traditional Home Health Care Pediatric Division is seeking a compassionate Licensed Practical Nurse (LPN) to join our in-home care team. We offer flexible shifts, competitive pay, and opportunities to work with pediatric and adult patients in the comfort of their homes.

You'll provide one-on-one care, administer medications, document care plans, and collaborate with clinical managers to maintain high-quality patient outcomes; weekend and evening options available to fit your schedule.

Qualifications

• LPN license required.
• Home health care experience preferred.
• Ability to provide one-on-one patient care.

Responsibilities

• One-to-One patient care in home/school environment.
• Implement and document the plan of care to ensure quality and continuity.
• Administration of prescribed medication, treatments and therapies.
• Patient assessments; coordination of care.
• Reports changes in client condition to Clinical Manager/physician.
• Health promotion, teaching and training of family members.

Shift: Weekday and weekend shifts, full-time and part-time schedules available!

Rate: $28-$32 per hour

Immediate Need: Sunbury, PA: 1: Thursday &Friday 6:30pm-6:30am - Saturday &Sunday 10:30pm-6:30am

2. Thursday & Friday 6:30pm-6:30am - Saturday & Sunday 10:30pm-6:30am

Sunbury, PA: Monday - Friday, school accompaniment, 8:00am - 3:00pm/ShikellamySchool District

Sunbury, PA: Trach/Vent overnight shift available

Dewart, PA: Wednesday 7:00am - 6:00pm/Tuesday, Thursday, Friday, Saturday & Sunday 6:00pm - 11:00pm
