Job summary

Traditional Home Health Care’s Pediatric Division is seeking a compassionate Licensed Practical Nurse (LPN) to provide one-on-one nursing care in patients’ homes and in school settings. You’ll assess needs, administer medications, and implement the plan of care with attention to safety and comfort for pediatric and adult clients.

Qualifications

• Must hold a valid LPN license and be in good standing.
• Experience with pediatric and adult home health care preferred.
• Flexible scheduling including days/evenings/weekends.
• Reliable transportation may be required.

Responsibilities

• One to One patient care in home/school environment.
• Implement and document the plan of care to ensure quality and continuity of care for patient.
• Administration of prescribed medication, treatments and therapies.
• Patient assessments; coordination of care.
• Reports any changes in the client condition to Clinical Manager and physician.
• Health promotion, teaching, and training of family members.

Shift: Weekday and weekend shifts, full-time and part-time schedules available!

Rate: $28-$32 per hour

Immediate Need

Sunbury, PA: 1: Thursday & Friday 6:30pm-6:30am - Saturday & Sunday 10:30pm-6:30am

Sunbury, PA: 2. Thursday & Friday 6:30pm-6:30am - Saturday & Sunday 10:30pm-6:30am

Sunbury, PA: Monday - Friday, school accompaniment, 8:00am - 3:00pm/Shikellamy School District

Sunbury, PA: Trach/Vent overnight shift available

Dewart, PA: Wednesday 7:00am - 6:00pm/Tuesday, Thursday, Friday, Saturday & Sunday 6:00pm - 11:00pm
