Job summary

Traditional Home Health Care is seeking a compassionate Licensed Practical Nurse to join our home-based care team in Bloomsburg, PA. We offer weekday and weekend shifts, full-time and part-time schedules, and the chance to provide one-to-one care to pediatric and adult patients in their own homes.

As an LPN, you will administer medications, implement the care plan, perform assessments, and collaborate with the Clinical Manager and physician.

Qualifications

• Must hold a valid Licensed Practical Nurse (LPN) license.
• Experience in pediatric and adult home health care preferred.
• Flexible scheduling and willingness to work various shifts.

Responsibilities

• One to One patient care in home/school environment
• Implement and document the plan of care to ensure quality and continuity of care for patient
• Administration of prescribed medication, treatments and therapies
• Patient assessments; coordination of care
• Reports any changes in the client condition to Clinical Manager and physician
• Health promotion, teaching, and training of family members
