Traditional Home Health Care Pediatric Division is seeking a Licensed Practical Nurse (LPN) for daytime/nighttime home care in Mifflinville, PA. The position offers weekday and weekend shifts with both full-time and part-time schedules.

The rate is $32 per hour, with care provided in patients' homes. Responsibilities include one-to-one patient care, administering medications and treatments, and coordinating with the Clinical Manager to ensure high-quality, continuous care for pediatric and adult

Qualifications

• Licensed Practical Nurse (LPN) licensure and current state credentials.
• Compassionate, patient‑focused care in home settings.
• Flexibility for weekday and weekend shifts.

Responsibilities

• Provide one-to-one patient care in home or school environments.
• Implement and document the care plan to ensure quality and continuity.
• Administer prescribed medications, treatments and therapies.
• Conduct patient assessments and coordinate care with the clinical team.
• Report changes in patient condition to Clinical Manager and physician.
• Educate and train family members on health maintenance.

Responsibilities
