Traditional Home Health Care Pediatric Division is seeking a compassionate Licensed Practical Nurse (LPN) to provide high-quality, one-to-one home health care for pediatric and adult patients in the Honesdale area. Weekly pay with direct deposit, paid time off from day one, and flexible weekday and weekend shifts (full-time and part-time) are available.

Qualifications

• LPN license required.
• Experience in home health care preferred.
• Ability to provide care in home settings.

Responsibilities

• One-to-one patient care in home environments.
• Implement and document the plan of care to ensure quality and continuity of care.
• Administration of prescribed medication, treatments and therapies.
• Patient assessments; coordination of care.
• Report any changes in client condition to Clinical Manager and physician.
• Health promotion, teaching, and training of family members.
