Traditional Home Health Care Pediatric Division in Forest City, PA is seeking a compassionate Licensed Practical Nurse (LPN) to provide one-to-one skilled nursing in the home for pediatric and adult patients. The role offers weekday and weekend shifts with open hours, and a supportive team environment focused on restoring health and independence.

Qualifications

• Licensed Practical Nurse with current license to practice in PA.
• Experience in home health care and pediatric settings is preferred.
• Compassionate, reliable, and able to work flexible shifts.

Responsibilities

• Provide one-to-one patient care in home or school settings.
• Implement and document the plan of care to ensure quality and continuity of care.
• Administer prescribed medications and therapies as ordered.
• Assess patients and coordinate care with Clinical Manager and physician.
• Report changes in client condition in a timely manner.
• Educate and train family members on health management.

Shift:Weekday and weekend shifts
Rate:$28-$32 per hour
Immediate Need:
Forest City:Monday - Friday 6:00am - 8:00pm open shifts/Weekend shift 8:00am - 4:00pm
