Job summary

Traditional Home Health Care Pediatric Division is seeking a compassionate Licensed Practical Nurse (LPN) to join our team providing one-on-one in-home and school-based care for pediatric and adult patients. We offer flexible scheduling, weekday and weekend shifts, and opportunities for full-time or part-time work.

As an LPN, you will administer medications, document care plans, assess patients, and coordinate with the Clinical Manager and physicians.

Qualifications

• Licensed Practical Nurse (LPN) license required.
• Experience in home health care preferred.
• Compassionate, capable of one-on-one patient care.

Responsibilities

• One-to-One patient care in home/school environment
• Implement and document the plan of care to ensure quality and continuity of care for patient
• Administration of prescribed medication, treatments and therapies
• Patient assessments; coordination of care
• Reports any changes in the client condition to Clinical Manager and physician
• Health promotion, teaching, and training of family members

Rate: $28-$32 per hour

Immediate Need: Cresco, PA: Friday & Saturday 7am- 9pm/flexible shifts available
