Job summary

Sweetwater Care in Fresno, CA seeks an LVN with nursing management experience to lead clinical operations in a long-term care setting. You will ensure care quality, coordinate with physicians, and oversee medication administration according to policy.

Responsibilities include infection control, quality improvement, patient and family education, and using Point Click Care for charting. California LVN license required; compact license acceptable where applicable.

Qualifications

• Licensed LVN in California or compact license if working in a compact state.
• Minimum one year nursing management experience preferred.
• Experience in long-term care required.
• Point Click Care charting experience preferred.

• Participates in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for patient placement and monitoring.
• Notifies other Departments of patient changes (room changes, LOA, admits, discharge).
• Conducts family and patient education.
• Administers medications timely and according to facility policy.
• Administers treatments timely and according to facility policy.
• Receives, transcribes, and carries out physician orders as allowed.
• Makes rounds with physicians when necessary.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
