Job summary

Sweetwater Care in Visalia, CA seeks an experienced Licensed Vocational Nurse (LVN) with management skills to oversee patient care within scope of practice, center policies, and state guidelines.

The role requires a California LVN license (compact license considered), long-term care experience, and familiarity with PointClickCare. You will lead nursing staff, ensure timely medication administration, participate in quality improvement, and coordinate with physicians.

Responsibilities

• Demonstrates knowledge and appropriate use of the Infection Control Manual.
• Understands, participates and assists in Quality Improvement Program implementation.
• Utilizes the Quality Life Manual for patient identification, placement, and monitoring.
• Notifies other Departments of patient changes (room changes, LOA, admits, discharge).
• Conducts family and patient education.
• Administers medications timely and per facility policy.
• Administers treatments timely and per facility policy.
• Receives, transcribes, and carries out physician orders when allowed by Nurse Practice Act.
• Makes rounds with physicians when necessary.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
