Job summary

Sweetwater Care in California seeks an experienced LVN to provide nursing care within scope of practice and ensure patient needs are met according to standards and physician orders.

This role supports the Quality Improvement Program, infection control, and patient education, with rounds alongside physicians as needed. California LVN license (compact acceptable) and long-term care experience are preferred.

Qualifications

• Licensed LVN in California; compact licensure acceptable in compact states.
• Minimum 1 year of nursing management experience preferred.
• Experience in long-term care setting.
• Proficient with Point Click Care charting.

Responsibilities

• Provide nursing care per standards and physician orders.
• Follow Infection Control Manual and Quality Improvement Program.
• Identify, place, and monitor patients in appropriate programs.
• Notify departments of patient changes (room changes, LOA, new admits, discharge).
• Educate patients and families.
• Administer medications and treatments timely and per policy.
• Receive and carry out physician orders as allowed by Nurse Practice Act.
• Participate in rounds with physicians when necessary.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
