Job summary

Sweetwater Care in California seeks an experienced LVN to provide nursing leadership within a long-term care setting. The role emphasizes clinical excellence, patient safety, regulatory compliance, and compassionate care for residents.

You will supervise clinical staff, coordinate comprehensive care plans, administer medications and treatments per policy, round with physicians, educate families, and partner to optimize resident outcomes.

Qualifications

• Active LVN license in California (or compact license if working in a Compact state).
• At least 1 year of nursing management experience preferred.
• Experience in long-term care settings.
• Experience with PointClickCare charting.

Responsibilities

• Demonstrates knowledge and appropriate use of the Infection Control Manual.
• Participates in implementing the Quality Improvement Program.
• Uses the Quality Life Manual for patient placement and monitoring.
• Notifies other departments of patient changes (room changes, LOA, admits, discharge).
• Conducts family and patient education.
• Administers medications timely and per facility policy.
• Transcribes and carries out physician orders as allowed by Nurse Practice Act.
• Makes rounds with physicians when necessary.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
