Job summary

Sweetwater Care in Los Angeles is seeking an experienced LVN with nursing management background to oversee resident care in accordance with policy and regulations. The role emphasizes leadership in clinical operations, patient safety, and compliance within a long-term care setting.

You will coordinate care plans, mentor nursing staff, ensure timely medication administration, and collaborate with physicians to support optimal patient outcomes.

Qualifications

• Currently licensed as LVN in California; compact license allowed in compact states.
• One-year nursing management experience preferred.
• Long-term care experience required.
• Point Click Care charting experience preferred.

Responsibilities

• Demonstrates knowledge and appropriate use of infection control policies.
• Participates in Quality Improvement initiatives.
• Utilizes care manuals for patient placement and monitoring.
• Notifies departments of patient changes and admissions/discharges.
• Conducts family and patient education.
• Administers medications timely according to policy.
• Administers treatments timely according to policy.
• Receives and transcribes physician orders as allowed by law.
• Makes rounds with physicians as needed.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
