Job summary

Sweetwater Care in Fresno, CA seeks an experienced LVN with management potential to provide nursing care in accordance with physician orders, center policies, and applicable guidelines.

Responsibilities include infection control, quality improvement participation, patient education, timely administration of medications and treatments, and collaboration with physicians as rounds are needed.

Qualifications

• Current LVN license in California or compact license if working in a compact state.
• 1 year of nursing management experience preferred.
• Long term care experience required.
• PointClickCare charting experience preferred.

Responsibilities

• Demonstrates knowledge and use of Infection Control Manual.
• Participates in Quality Improvement Program.
• Uses Quality Life Manual for patient placement and monitoring.
• Notifies departments of patient changes (room changes, LOA, new admits, discharge).
• Conducts family and patient education.
• Administers medications timely per facility policy.
• Administers treatments timely per facility policy.
• Receives/transcribes/executes physician orders as allowed by Nurse Practice Act.
• Makes rounds with physicians when necessary.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
