Job summary

Sweetwater Care in Tulare, CA is seeking an LVN with leadership experience to oversee nursing services within scope of practice and facility policies.

You will collaborate with physicians, conduct patient and family education, and participate in quality improvement, infection control, and charting with PointClickCare.

This role requires CA LVN licensure; prior management and long-term care experience preferred.

Qualifications

• Currently licensed as LVN in California; compact license if working in a compact state.
• One-year nursing management experience preferred.
• Long-term care experience required.
• PointClickCare charting experience needed.

Responsibilities

• Demonstrates knowledge and use of Infection Control Manual.
• Implement Quality Improvement Program.
• Utilize Quality Life Manual for patient placement and monitoring.
• Notify departments of patient changes (room changes, LOA, admit, discharge).
• Conduct family and patient education.
• Administer medications timely according to facility policy.
• Administer treatments timely according to facility policy.
• Receive, transcribe, and carry out physician orders as allowed by Nurse Practice Act.
• Make rounds with physicians when necessary.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
