Job summary

Sweetwater Care in California is seeking an experienced LVN nurse manager to oversee nursing operations in a long-term care setting. You will ensure patient needs are met per standards and physician orders while leading staff in daily rounds and care coordination.

The role requires an active California LVN license, compact license where applicable, and prior management experience. Expect routine collaboration with physicians, families, and interdepartmental teams to maintain quality care.

Qualifications

• Licensed LVN in California; compact license acceptable in compact states.
• Minimum 1 year nursing management experience preferred.
• Experience in long-term care required.
• Proficient with PointClickCare charting.

Responsibilities

• Demonstrates knowledge and use of Infection Control Manual.
• Participates in and assists in Quality Improvement Program.
• Uses Quality Life Manual for patient identification and monitoring.
• Notifies departments of patient changes (room changes, LOA, admissions, discharges).
• Conducts family and patient education.
• Administers medications and treatments timely per facility policy.
• Receives, transcribes, and carries out physician orders per Nurse Practice Act.
• Makes rounds with physicians when necessary.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
