Job summary

Sweetwater Care in California is seeking a nurse manager (LVN) to deliver nursing care within the scope of practice and ensure patient needs are met per standards and physician orders. The role involves medication administration, patient rounds with physicians, and family education.

Ideal candidate holds a California LVN license (compact license acceptable for other compact states) and has long-term care experience, with a preference for prior nursing management.

Qualifications

• Currently licensed LVN in California; compact license accepted for compact states.
• Minimum 1 year nursing management experience preferred.
• Experience in long-term care.
• Experience with PointClickCare charting.

Responsibilities

• Administer medications timely and according to facility policy.
• Administer treatments timely and according to facility policy.
• Receive, transcribe, and carry out physician orders where allowed by Nurse Practice Act.
• Make rounds with physicians as needed.
• Educate families and patients about care plans.
• Notify other departments of patient changes (room changes, LOA, admits, discharges).

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
• LONG TERM CARE EXPERIENCE
• POINT CLICK CARE CHARTING EXPERIENCE
