Job summary

Sweetwater Care in Bishop, CA is seeking an LVN with nursing management experience to oversee patient care in a long-term care setting. The role involves implementing infection control, quality improvement, and accurate medication administration according to physician orders and facility policy.

Candidates should be licensed as LVN in California (compact license if working across state lines), have long-term care experience, and familiarity with POINT CLICK CARE charting.

Qualifications

• Currently licensed LVN in California; compact license accepted if working in a compact state.
• Minimum one year nursing management experience preferred.
• Experience in long-term care.
• Proficient in POINT CLICK CARE charting.

Responsibilities

• Demonstrates knowledge and appropriate use of infection control policies.
• Participates in the Quality Improvement Program and implements its initiatives.
• Utilizes the Quality Life Manual for patient identification, placement, and monitoring.
• Notifies departments of patient changes (room changes, LOA, admits, discharge).
• Conducts family and patient education.
• Administers medications timely and according to facility policy.
• Administers treatments timely and according to facility policy.
• Receives, transcribes, and carries out physician orders as allowed by Nurse Practice Act.
• Makes rounds with physicians when necessary.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
